    very true @shashikantiyengar ji
    no one is able to solve this riddle.... we are blaming carbs... but I don't think carbs are to be blamed...
    It is true that as diabetic we are not efficient to clear carbs... so we should control carbs as far as possible..
    But I think problem is some where else...
    No one knows...
    It is like Vedic search for God... every one says Neti... neti... (not like this..not like this)

    But this signalling is lost when load the system with excess of carbs ..
    But as I keep saying .. don't know what led to this
    But what can lead us out is low carb
    :)

    Yes...low carbing will help us and give us relief....but I think that is not final solution....
    We must seek for that final solution...

    Loss of first stage insulin response cannot be reversed.

    So its IR + stage 1 insulin response lost...

    With low carb and by losing fats we can over come IR... but as rightly said by @Anup Singh we should think about this first phase insulin response...
    What we can do to regain this first phase response??

    I agree. There are many obese people who load up on carb, they may have hypertension, various cardiac ailments but are non diabetic. Everybody knows bad effects of tobacco but there are chain smokers who live long healthy life without getting any form of cancer. I am not saying that once diabetic, one should load up on carb. But there are various puzzles about human body that medical science has yet to decipher

    Earlier it was also believed that without medicine diabetes can not be controlled...
    And @Anup Singh did that...he is medicine free for last 6 yrs now... and non diabetic range...

    Why we are losing first phase insulin response ??? are they tired??? if so then giving them rest can revive them... IF is meant for that..
    Is it due to Apoptosis of beta cells then we don't see that in T2...as we see elevated insulin level in T2 patients...
    is due to malfunction of beta cells?? hmmmm what could be done to get them functional???
    Again are we still moving around insulin and beta cells???
    There has to be something else...

    We all know that he's controlling his t2d by eating foods that don't tax his insulin response.
    I'd rather be eating rice & potatoes, but I can't.
    He can't either.

    I read it somewhere that if you are able to overcome the IR by loosing fat(Visceral) and/or by any other means ,then to compensate for lost first phase insulin response one can inject him/her self with few units of long-acting insulin.
    But i guess that will be counterproductive as one is bound to get trap in vicious cycle
    @Anup Singh your thoughts on this ?

    That would be needed if on high carb
    With low carb this need is eliminated

    Try to mimic that by:

    Have a protein only stuff before main course of meal. Maybe an hour before the meal. See if that helps -- provided IR is zero.
    If it doesn't help, then might as well not break head too much on it. First stage insulin response gone == car running on weak batteries where you get many diagnostic alarms many a times -- airbag failure, engine failure, immobilizer failure. When the batter gets weaker, longer self needed to start the car. When one keeps running in such a condition, finally the starter fails. :)

    That extrapolation to what Bhagwan Krishna said is taking it too far IMO.
    The old car is still working, though in a reduced capacity.
    It doesn't have to be consigned to the scrap yard just yet.
    It can still run, as is proved by all of us T2D guys, if we exercise some self control on what we eat.

    There are several factors which affect insulin sensitivity.
    It is difficult to make exact reason for IR and it also varies from person to person, so different food have different effect on different person.
    Some can tolerate mango while many other can;t.
    Genetics plays an important role here.
